(valid email required) is attached to a name field and I cant seem to get it disassociated. I have deleted the field and re added a name field, but it still is associated with an email address field.
any help on this?

config looks ok.

it seems that you have added form #2, does this also happen with your default form?

or a new form (form #3)?

you could also try to uninstall the plugin and start from scratch.
