Did you check the FAQs?
E.g. "empty line" at the end of cforms.php etc.?

Hi, i had similar problem a few weeks ago but i think i found out my problem. I number the WP style sheet filename as i update it to prevent caching and had deleted the default style.css file while tidying up. When opening the WP Design page, the template preview will fail because it can't find the style sheet and WP disables the template, but there is no other template to use so the site goes blank, while the admin remains functional
When installing CForms the same check for style.css may be triggered and so the template is disabled. I have now got 9.3 running and it seems fine so it may have been a 9.2 issue but I think in my case it was the missing default style sheet. Check the Design page in WP to see if your template is ok if you get the blank site
