This is really wierd and it's quite irritating… Im not sure if you could help but I might as well try. I've searched around (using search funcion and reading the FAQ) and didnt find anything similar…

I have 2 pages with fully functional forms (page1) (page2), however if I created another form, or use one of those same forms on ANY other pages with the same settings, it shows up like this (click)

I've tried deleting all my forms and starting a new one, it still only displays correctly on these pages (page1) (page2). I have also tried creating new pages and it is still doing the same thing.

The first screen is the working form, the second screen is one of the non working pages. Particularly the one I linked above..

it appears that on the pages with the broken forms, the cforms header is not loaded.

check your global-settings / cforms header
