I'm working on this particular form with the "using pre formatted form" option checked and i couldn't make some of the values show on the admin email.
Sample form

Present Address: – single line of text -
Telephone Number: – single line of text -
Permanent Address: – single line of text -
Telephone Number: – single line of text -
=====================================
I know "Telephone Number" is the offending field because it's not unique. But is there a way to go around with this without changing the field name?
I'm thinking
{Telephone Number [0]} – value on the first tel. no. field
{Telephone Number [1]} – value on the second tel. no. field
Any help would be appreciated.

Generally, I don't see a reason why not to use the same field name, UNLESS you're intending to use custom variables in the email 'header' and thus would need a unique identifier (the field name) to reference it.
PS: Not sure if you're using ':' in your field names, I've been told by another user that that might cause issues…not confirmed though.
