"page not found" when submitting with file upload feature

hello,

 I have finally figured out with the help of the hosting provider the proper path i should be using for the upload directory.  It turns out that it a windows server and after long back and forth, i set the upload path so that i don't get the error message anymore " directory does not exist" however now, when i actually try to upload a file and press submit button, i get the Page not found !

 

Please direct me in the right way.

This may be related to a "redirect bug" of 4.8 that kicks in when the actual mailing fails. However if you do get the form email (with attachment) and the attachment is now being saved, then I'm stumped.

I'll send you v5.0 (beta! please ignore all the new HTML stuff in the message(s) config area), let's see if this either fixes the issue or helps us to track down the error.
