This form should populate the database with the 38 resposnses.

The database (and the email i get) stops at this question:

Will you be enrolling your dog/puppy in obedience classes? If no, why not?

It also gives the answer to that question as "X", and not the correct response.

I have no idea what is going wrong here any help would be greatly appreciated!

Mike, thanks for the detailed info.

It seems that some of the field labels are too large for being recorded in the Tracking DB, not sure if this causes subsequent errors. Currently the Traking DB  is limited to labels with 100 Characters.

I sent you an email. 

PS: I would also use a different setup/layout, since some of the labels could be inserted as "text-only" above a field and then have a shorter counterpart instead. It would make it more compact. A few CSS changes (width) could also enhance the look & feel.
