I have set up the form, and and looks well.  It is appending to the tables in the database properly as far as I can tell.  I created a form field with subscribe/unsubscribe radio buttons.  When I fill the form out, it simply sends emails and adds rows to the database with unsubscribe or subscribe in the field.  So how do I tell it to remove someone from the mailing list if they choose unsubscribe? 

Or would it be better to create a separate form for someone who wants to unsubscribe with only email address and unsubcribe so that they are not required to fill out the other fields?
