Is there anyway to have a checkbox that say same and when checked it copies the information from fields already filled in above it?

im planning to ask for an address, city, state, and zip. and then an emergency contact address, city, state, and zip and both are required but i would like to be able to have a check box that when checked fills in the information from the first address.

you could do it via Javascript (jQuery). there are some threads in this forum that generally cover check-box triggered form modifications
