Great plug-in, thanks Oliver.

 Newbie question – my form is running fine. But I don't need two of the fields. I have disabled them but they still show.

That's actually the purpose of the "disabled" feature. To show that the form normally offers this particular input field, but it's currently disabled. It's a usability thing.

If you like to get rid of the two fields, just hit the red X. You can backup the form prior to that just in case you reconsider…
