Note, the imporper styling on the date picker, and cannot adjust by year, month only.

 

Also, the form refreshes the page on submit, even though ajax is enabled.

 

I had cforms 12.x on there before. I uninstalled / deleted all cforms data using the button in the global settings, then removed the deactivated cforms v12 plugin, and uploaded the latest, v13.
