Hey all, I am wondering... I need to make a form that  all which have multiple lines of text following the field, easy.  Thats the simple part! 

Now I would like when a specific field is filled in with the users data to link to preformated text on the DB and be emailed along with the form data upon submition.  so if field "A" is filed in it will link and include preformated text from the DB cell "A" field "B" linkedwith DB cell "B" and so on.

Any Ideas?  I dont really do DB work so im not sure how this would work.
