Duplicating uploads
Submitting a form with multiple image uploads, duplicates the last one

Hi, We have a form that when it is submitted (mostly via mobile as far as we can tell) duplicates images that are uploaded.  For instance, if you upload 3 images, you'll get the last image duplicated 3 times and not the first 2 images.  Also, if you upload 5 images, you'll get the 5th image duplicated 5 times and not the first 4 images.  We can't at all figure this out.  Has anyone come across this before?
