displays on every page..need help

this issue was briefly mentioned in the FAQ but showed no specific resolution. on my site – ipholio.com/about/ – it displays my contact form perfectly. however, it displays cform on every other page on my wordpress. i'm currently using 4U Hemingway for my theme and have no clue what to edit to keep this problem from occuring. any help would be extremely appreciated.
