Having the same problem, which occurred when upgrading from 13.x to 14.5 on WP 3.4.1. Troubleshooting so far hasn't turned up anything.

Further troubleshooting has revealed a conflict with the WPaudio plugin (http://wpaudio.com/). Jude, have you tried disabling all of your plugins, then re-enabling them one by one?
