I have installed and activated the plugin and added it to a page but it does not work. I do not get the submissions. If I scroll all the way down to the bottom of the WordPress dashboard, I get the following Database Error

 

SELECT * FROM wp_cformssubmissions WHERE form_id in ('') ORDER BY sub_date DESC LIMIT 0,5

 

Any help would be appreciated!

Can you please post your URL?

'added it to a page but it does not work' is difficult to trouble shoot :-)

EDIT: You need to Enable Database Tracking to get rid of the error on the dashboard.
