I am trying to build a custom form where when someone selects a certain option for a drop down box it would add new fields to the form but cannot for the life of me figure it out.

I want people who go to http://iammiak.com/index.php/contact/ and select the get your band reviewed to be sent to this form http://iammiak.com/index.php/submission/

Can someone tell me how to do this?
