Perhaps. So you say your default form works but an additional one doesn't?
The only explanation I have is due to your admin email settings. You may want to compare all settings between the two forms configs.

It looks good from the user point of view.
- Have you checked your SPAM folders on your admin email account?
- Can you use a different admin email address for testing?
- Can you get in touch with your provider and ask them about the not delivered emails?
- They should be able to see immediately why an email hasn't been delivered.

hi i have checked the spam email. by the way i am using gmail account to test, before it was a personal email which does not work so i tried it on gmail but same thing.
could this be because wordpress is installed manually at the root? this problem doesnt happen wt other website which has automatic wordpress installation to root.
but thats just cause this is the 1st this happen on this particualr website.

Again from the front end, all seems to be working fine. So I'm sure there are no deployment issues, but configuration/mail server issues.
Are you using cforms SMTP support?

No you shouldn't have to, unless your providers makes it mandatory.
That leaves only points 3) and 4) above.
