Captcha & Visitor Verification broken in cForms 12.2

Hello,

 

I just installed a fresh copy of WordPress 3.2.1 along with the new cForms 12.2 and noticed that captcha images and visitor verification fields do not display in the form, and are not in View Source in Firefox 3 or 5. However, in Chrome and Internet Explorer the fields display, and are displayed in the View Source. Given that the output is just not even present in the Firefox source I'm thinking this issue is a bug – as this would be beyond and local style or javascript. I've tried switching WordPress themes and still no luck – the captcha / verification fields are just not even being generated. 

 

**EDIT** – CORRECTED – By default the 'Force Q&A display for logged in users' option is left unchecked :( So you can add verification fields, and bang your face on the floor when they don't show up… until you log out : :

Thanks for the edit- LOL- I was able to solve my frustration with this same problem with minimal brain damage thanks to your post.

Thank you as well ! This issue was driving me crazy! After reading this, I realized that I needed to add a new verification field to my form.

I inherited a Wordpress site with lots of plugins installed.  Started having broken Captcha image after Wordpress and updating all plugins.  I suspect my issue was caused by another plugin adding white space to the output buffer, however the solution that worked for me was to change CFORMS-CAPTCHA.PHP

Added ob_clean() at line 176:

### a nice border
ImageRectangle($im, 0, 0, $image_width-1, $image_height-1, $color_border);
ob_clean();
switch ($output_type) {
case 'jpeg': Header('Content-type: image/jpeg'); ImageJPEG($im,NULL,100); break;
case 'png':
default: Header('Content-type: image/png'); ImagePNG($im); break;
}

Like I stated I do not think it's an issue with the CFORMSII plugin, but rather some other plugin.  Changing the CFORMSII plugin was simply easier and acceptable for my needs.
