Please replace the ??? with your data!
I've got two forms on the same website (the other one is at http://ibl-idiomas.com.br/?page_id=30). Both "Admin Email Message Options" sections are set exactly the same way. On both the line "Admin email address(es)" have the same list of e-mails. However, the first form just stopped sending forms to addresses other the first one.
In other words, the one at http://ibl-idiomas.com.br/?page_id=5 now sends data only to the first e-mail of the list and ignores the rest. The other one sends to all of them.
What could have happened?

it appears to be the exact same form on both pages (form #2)?! is really working differently on both pages?
- are you using my-functions.php?
- a Multi-Recipient field?
- the TAF option?

Ops! You're right, the forms were the same. But they're now fixed, maybe you'd like to recheck them:

Now answering your questions: all answers are no, even because I don't even know how to use them. What I do on some forms is setting more than one e-mail address at "Admin email address(es)" separated by comma. Usually it works fine except on that one — and at firts it also worked well but, for some odd reason, it stopped working as I described on my first post.
I'd appreciate your comments on this issue.

Might want to start with a simple (default) form and one admin email address, then try the other, then together.
Are you using BCC?

second form uses a "Multi-Recipient" field, hence only the selected choice/admin email is used.

I see. I can't remember setting a Multi-Recipient field…
