our online application form was working for a month or two, but suddenly is not working, l although i had not changed anything. upon SUBMIT, it generates a 404 Not Found error. Today, i have changed a few things. checked the 

var sajax_uri = 'http://www.hphospice.org/janky/wp-content/plugins/cforms/lib_ajax.php';

added the full URL.

emptied cache; restarted browser.

i also removed the upload resume part of the application thinking that that was causing trouble.

no joy.

thank you, oliver. i got your test application. 

i ended up just making a duplication of the form with a new name and updated that page with the new cform. not sure why i had to do that! i should have updated this thread, but hadn't had a chance yet.
