This is, by far, the best contact form on the planet. (And I would know; I invented all of the other ones. jk, of course.)
Honestly, this is exactly what I've been looking for, and more. I've used it on the NorthWest Down Syndrome Association's site (http://nwdsa.org/resources/) but I certainly won't stop there. I absolutely love the customizability and fullness of it. Took me a while to figure out that I shouldn't use colons (":") in the field names but, other than that, it's been smooth sailing.
Also, I wanted to comment on your site - it's definitely one of the nicest I've seen, and the fact that it's built with WordPress makes me a little giddy. lol
