how to set the value for the checkbox?

I want to add a checkbox to the form when the user agrees that sending a copy of the completed form to his e-mail, which is taken from the boxes on the form. But I could not find how to do it, how to set a value to this checkbox?
The general form of installations have the option to send a copy of the user, but in a different way.
First, this option for all the generated forms.
Secondly do not want to clog mail without the user's wishes.

Is there a solution?
