Defining Etiquette Sidebar Form

[desc]:  I'm using the form to allow visitors to submit "Etiquette Requests" to the world in my sidebar.  I have it set up so very little information is required–for now.  It also just sends me an e-mail and I post the info, so my site will stay clean.

I thought I better mention that this is no longer a sidebar form.  I decided a form in the sidebar wasn't right for my site.  Instead, I used the minimal stylesheet and fidgeted with it until it looked alright.  It varies from computer to computer, but I think it generally looks okay.  Hopefully I'll get a little better at programming and make it look nice eventually…
