_I recently updated this plugin to the latest version and it took away the contact form on the other pages on my blog.  I have wp version 2.6 and the only change I have made is to update this plugin.

Any suggestions?

I am having this problem too. FANTASTIC plug in by the way! My test worked great, however, when I go into global settings to add a page (I'd like it to show on my services page as well), it numbers each of the items on the form and the form shows on the home page only.  I tried to find an answer in Faqs but didn't come up with anything.
