We have a contact form using 'tell a friend'
- how do we make it show on one 'page' only?

Must it be added to the page.php template?

Can you pls share your URL?! 

I suggest you make a copy of the default theme's page.php template, add a custom header (see WP documentation on how to set up custom page templates) and select it as your page template when creating a new page in WP.

The T-A-F code then only goes into this new, custom page template.
